Problem with my fan / heatsink

I have had my laptop (Acer Aspire 5920) for 3 years with no problems. Recently it has been making a loud buzzing noise ,coming from the fan, when windows is starrting up, but then the noise stops after about 2 minutes. I cleaned the fan and the noise stopped for about a week. But now the noise is back and doesn't go away. Cleaning the fan doesn't work. Is this a problem with the heatsink? What should I do to fix it?
